Okay, reasonable training video.
The question about whether a company should have an accident book, I got wrong. Legally, this is not necessary for every company but, to help with potential claims and have a document repository for the history of such things and for best practice, I don't agree that they shouldn't. Had it put "legally speaking…" I would not have given the wrong answer.

overall good information
overall good content, be good to mention ladder ratings from light duty up to heavy duty and when use is appropriate for task, as well as when to use say a fiberglass ladder for electrical work
