Good course, some answers not clear cut
The question asking if all organisations need to have an accident book is not specific to working at height and the law specifies that all employers must report and keep records of work related accidents. An organisation is generally associated with a company of numerous employees. Also, when asking about taking things likely to fall up a scaffold the first incidence of safety should be to eliminate the risk and take an alternative not likely to fall also signage is a less robust solution, the area should ideally have barriers to prevent access.
